What Is a Medial Branch Block?

A medial branch block (MBB) is a test performed under fluoroscopy to help temporarily relieve pain caused by facet joints by using local anesthetic agents. 

The primary goal of an MBB is to confirm the facet joints as the source of pain and determine if a patient is a suitable candidate for radiofrequency ablation (RFA), a longer-lasting treatment for managing facet joint pain.

What to Expect with a Medial Branch Block

A medial branch block (MBB) is used to identify and temporarily relieve pain originating from the facet joints in the spine. It involves injecting a local anesthetic to numb the medial branch nerves that transmit pain signals from the facet joints to the brain.

The medial branch block procedure consists of 2 appointments per treated site, at least 1 week apart. Dr. Marro, our Peace Diagnostic Imaging in-house specialist, will perform the MBB procedure.

Patients who see a significant improvement in their pain will be considered a suitable candidate for RFA. We will inform your referring doctor to arrange the RFA procedure with Dr. Marro.

  • Preparation: The patient will lie on their stomach, and the area of concern will be cleaned with an aseptic solution.
  • Injection: A small needle is guided to the appropriate nerves using fluoroscopy (live X-ray). An anesthetic (Lidocaine or Bupivacaine) is injected around the nerve.
  • Duration: The procedure takes approximately 5-10 minutes, followed by a 10-15 minute monitoring period.
  • Follow-up: Patients are given a pain diary to record pain levels over the next 24 hours. The procedure may be repeated with a different anesthetic for further assessment.

Most patients can resume normal activity after the testing. However, injections are invasive procedures. Any invasive procedure has risks, side effects, and possible complications. Complications are rare but can include:

  • Allergic reaction
  • Temporary weakness or numbness
  • Bleeding, redness, or infection in or around the injection site
  • Fever or inflammation

What Is Radiofrequency Ablation?

Radiofrequency ablation (RFA) is a pain management treatment that uses radio waves to generate heat and interrupt pain signals from the medial branch nerves in the spine. 

RFA aims to provide long-lasting pain relief by destroying specific nerve tissue that transmits pain signals from the facet joints to the brain.

RFA can be especially effective for long-term pain relief in patients with chronic neck and lower back pain that hasn’t responded to other treatments.

Frequently Asked Questions

Medial branch nerves are very small nerve branches that carry pain messages from the facet joints and the muscles around the joints to the brain. If the nerves are blocked or numbed, they cannot transfer the pain sensation from the joints to the brain.

The facet joints are small joints in the back of the spine that form connections between each vertebra. Each facet joint has 2 nerves.

Typically, an MBB procedure takes about 15–30 minutes.

If the facet joints are the source of pain, you should benefit from the injection immediately. Depending on the medication injected, the effect can last up to 24 hours.

If you benefit from the MBB procedure, we will send a letter to your doctor indicating if you are a good candidate for Radio Frequency Ablation.

Serious side effects and complications are uncommon. The most common problem after the injection is discomfort in the area for a few days. More rare complications may include an allergic reaction, bleeding, or infection in or around the injection site, and temporary weakness or numbness.

If you have an active infection, you should not have the procedure. If you are pregnant or actively trying and it is 10 days past the first day of your last menstrual cycle, you should contact the clinic to see if you are able to proceed. 

Please warn us of any allergies, especially to local anesthetics, X-ray dye, and latex.

The MBB procedure will be done at Peace Diagnostic Imaging, suite 108, 9815 97 St, Grande Prairie.

We would highly recommend it. On occasion, freezing can affect the nerves in the legs, making you unfit to drive.

Complications are rare but possible. Feeling some increased pain on the day of or after your injection is normal. Depending on the injection site, you might also feel numbness in your leg or arm, but this usually fades within 24 hours. 

If your pain increases or you have a fever, chills, or signs of infection, contact your doctor immediately or call 811.
